1. Minimize Sun Exposure

If you’ve undergone aprocedure like a facelift or rhinoplasty, it’s very important to be aware of the amount of time your spending outside in the first few months. Though this time should be limited, if you do need to spend time outdoors it’s crucial to apply sunscreen of at least SPF 30. This helps ensure that the suns harmful rays will not do damage to the skin or discolor the incisions.

2. Use Mild Products

In the weeks following your facial plastic surgery, it’s a great idea to baby your skin and only use products that are hypoallergenic and fragrance free. This helps to make sure you’re skin won’t become irritated and will reduce the amount of redness that you might be experiencing post-procedure.   At my office, we have a line of image skin care products that are specifically designed to be used after surgery.  We also carry a line of mineral makeup that is both safe and healing to skin after surgery while covering up any bruising that may be present.

3. It’s All About H20

With the holiday season just behind us, it’s now time for members of Hollywood to get into tip-top shape and achieve a healthy facial glow that most people think is unattainable. However, as a facial plastic surgeon trained in Beverly Hills, I perform the very same treatments that celebrities will be seeking shortly before awards shows like the Oscar’s, the Emmy’s, and the Grammy’s at my office in Charlotte. I want to let my patients know how they too can achieve a similar look with my top three picks for this year’s award season.

1. Facial Fillers

At this year’s award shows, you can expect to see plenty of celebrities with refreshed looking faces due to facial filler injections—most commonly hyaluronic acid fillers like Juvederm or Restylane.  Important Questions To Ask Your Facial Plastic SurgeonAs a facial plastic surgeon in Charlotte, I perform cosmetic surgeries for my patients on a daily basis. However, one of the most important steps that occurs before a person ever undergoes surgery is the initial consultation.   During the consultation, I listen to the patient’s questions and concerns, and I educate my patients on the aging process.  I then explain where they are in the aging process and then I give them all of the treatment options, both surgical and non-surgical.  I encourage them to ask me any questions they might have about the procedures, so that they have a clear understanding. I find this places patients at ease as they know what to expect in regards to the process and anticipated goals.

1. Are my ideal results realistic?

 Facelift, Neck Lift, or Both?The decision to undergo a facelift or a neck lift should be taken seriously and should require a great deal of investigation and research.   As a facial plastic surgeon in Charlotte, I believe that my job is to educate my patients on all of their options, their benefits and risks, and allow the patient to make the ultimate decision.  In this blog, I’d like to discuss options for correcting an aging face with facelift surgery, neck lift surgery, or combining the two procedures.

Differences Between a Facelift & Neck Lift

One of the most common areas to show age on our faces is the loss of the jawline and development of excess tissue underneath the chin.  Methods of improving these areas are 2 of the most common surgical procedures, the facelift and neck lift.  The operations to improve the neck and jawline are customized to meet the unique needs of each patient.

A facelift aims to correct aging in our  lower face, jowls, and jawline, where sagging skin can leave us looking older than we really are.  An attractive jawline is a smooth uninterrupted line that gently curves into the chin.  When this line becomes wavy or blunted, it takes away from the definition in the face.  Alternatively, a neck lift works more to fix the appearance of a “turkey neck” or sagging neck underneath the chin.  This occurs as a result of skin laxity and excess fat deposits.

Many times, the best treatment option is to undergo a facelift and a neck lift at the same time. Because these two areas are very intimately related, undergoing both procedures can make for a more balanced facial appearance that will rejuvenate the entire lower facial area.

Mother Daughter Facelift 300x199 Like Mother, Like Daughter There’s an old saying that goes, “Like mother, like daughter.” If recent cosmetic facial plastic surgery trends are correct, this saying is becoming increasingly relevant.   Just recently, there has been an increase in mother-daughter duos coming to my office in Charlotte for a consultation. During the consultation, both the mother and the daughter get to share a unique type of bonding that’s fun, feels good, and can also make them look great!

Mother-Daughter Surgeries

Going through the plastic surgery process together can actually make people feel much more at ease and comfortable with their procedure, as they know that they have someone there to support them throughout the entire process. Traditional Facelift

An older woman with more advanced signs of aging may elect to undergo a surgical facelift, which is performed under general anesthesia and as an outpatient procedure. Before undergoing the procedure, I make sure to address any and all questions or concerns that my patients may have towards their surgery. During a traditional facelift procedure, I make incisions in the temple and the hairline, where they are then hidden inside the ear or in its natural crease. The goal of a traditional facelift is to lift sagging tissue, remove excess skin or fat, and add volume when needed.  I commonly advise patients with  fine lines, wrinkles, and other skin imperfections to undergo a medical grade chemical peel to improve the appearance of the skin.

Following the procedure, patients can expect some bruising and swelling for the first couple of weeks, but all should subside in the months following your surgery, leaving you with fantastic and natural looking results.

How Old Do You Want to Look?

Have you tried everything you can to look younger? Have you changed your diet, colored your hair, bought the latest skin firming cream or gotten a chemical peel? All these things can definitely help gain you a year or two, but you might not get the dramatic results you crave. If you have deep-set wrinkles in your forehead, sagging skin or perpetually puffy under-eyes, a face lift might be worth looking into. In Charlotte’s competitive work atmosphere, it can give you an edge by turning back the clock seven years.

Seven Years is a Long Time

In a new study, published in February 2012, researchers sought to find out just how many years younger people looked after undergoing a face lift. Sixty patients between the ages of 45 and 72 had various forms of facial plastic surgery.  Patients were placed in groups depending on the amount of procedures completed – with group one having the least amount of procedures and group three undergoing the most. Before and after photos were polled by a group of first-year medical students, who gave approximate ages for each picture.

The Award for Youngest Looking Goes to….

The results of the study found that all participants looked younger post-face lift surgery, with the average age approximation being 8.9 years younger than chronological age. The three surgery groups’ results did differ, however. Patients in group three, who had the most procedures done, seemed to reap the greatest benefit, as the judges awarded them with the largest age gap when compared with the other two groups.

Types of Face Lifts

Now that you know a face lift can literally erase seven years of wear and tear on your face, you might wonder what face lift options you have in Charlotte and other cities in the U.S. No two face lifts are alike, states the American Society for Aesthetic Plastic Surgery, but the goal is typically the same – to look younger without appearing fake or “plastic.” Discussion and evaluation with a cosmetic surgeon is essential; you have to agree on what you want done and what actually can be done during your face lift. Your Charlotte-based surgeon may suggest eyelid surgery to reduce a “droopy-eyed” appearance or he might opt to tighten the skin around your jaw and chin to create a younger looking profile.

Do Your Research

Make sure you check out all the cosmetic surgeons in Charlotte before you get a face lift. Finding the right doctor to work with is important; you wouldn’t trust your face to just anyone. You also need to understand the procedures you’ll have done, and the risks associated with the surgery. Doing it right the first time is important, as botched face lifts are difficult to remedy.

Why I Recommend a Chemical Peel During Facelift Surgery

A facelift surgery is a powerful procedure to reverse the signs of aging, which can be tailored to fit the unique goals of the patient.  There are a few basic goals of a rhytidectomy (facelift surgery):

1)  Fix a turkey neck (sagging neck skin under the chin)

2)  Improve definition of the jawline

3)  Raise the sagging skin of the face

Many patients believe that a facelift will improve the fine wrinkles on their face, but this is not true.  Unfortunately, a facelift will not improve the quality of the skin and therefore will not improve the fine wrinkles on the face.

What is the role of a medical grade chemical peel during face lift surgery?

A medical grade chemical peel can be performed simultaneously during a facelift to improve the quality of the skin and improve the fine wrinkles on the skin. When done in conjunction with a facelift it is the best way to improve overall facelift results and overall downtime.

What medical grade skin peel does Dr. Kulbersh use?

I prefer a 25% to 35% concentration of TCA (Trichloroacetic acid) peel.

Where are the peels applied?

In my facelift surgery technique, the peel cannot be applied to the entire face.  Applying the TCA peel over the tissues of the facelift can be too strong for the skin and may not be safe.  The common areas to apply the peel during a facelift are around the mouth and eyes.  These areas commonly have abundant fine wrinkles that are concerning to the patient.

Can a TCA peel be applied to the entire face?

I commonly apply the peel to the entire face when the patient is not undergoing a facelift at the same time.  This procedure can easily be done in my office.

Can the peel be applied during a necklift, browlift, or eyelift?

Yes, with a similarly positive effect.

Why Is It So Important to Stop Smoking Before a Facelift, Nose Job or Any Facial Plastic Surgery?

At Carolina Facial Plastics, nothing is more important than your health.  The risks of smoking are well known, and are increased risk of stroke, heart attack, cancer, and lung disease, but smoking will also dramatically increases your chances of a complication after a facial plastic surgery procedure.  The nicotine in the tobacco attacks the small blood vessels in your body resulting in poor circulation.

Smoking effects the heart, lungs, and blood vessels, and also complicates the anesthesia during a facial cosmetic surgery procedure.  It may be harder for the anesthesiologist to control your blood pressure or may effect the medications they use.  Anesthesia is done safely everyday on people who smoke for non-elective procedures, but for elective cosmetic surgery, at our surgery centers in Charlotte and Columbia, we do not believe in taking any undo risks.

During a facelift, or rhytidectomy, the skin is lifted and redraped to create a tighter, smoother, and refreshed face.  The major blood supply in the skin is the small blood vessels that are attacked by nicotine.  If the skin does not receive enough nutrients through the blood, the skin may die.  This then requires intensive care and results in a less than ideal outcome.  In my opinion, for any surgical procedure this is not acceptable.  Studies show that smoking increases the chances of the skin dying by over 20 times!

An open rhinoplasty, or nose job, requires lifting of the skin of the nose to be able to manipulate the underlying bone and cartilage to shape the nose.  The skin of the nose is also supported by the small blood vessels that are affected by nicotine.  Again, smoking places an unacceptable increased risk on the nasal skin for poor healing.  This risk is even greater in a person undergoing a revision rhinoplasy, or a repeat nose job.

As a board certified head and neck surgeon and a fellowship trained cosmetic surgeon, I believe in safety first, and am extremely cautious in treating patients that smoke tobacco.  The literature states that patients undergoing elective cosmetic procedures should stop smoking for at least 4-8 weeks before surgery.  It is well known that your body has the ability to continue to heal itself ever after 8 weeks.  If you have even a 1% chance of improved outcome for waiting longer, then why not wait?  I always think, what would my recommendation be to my mother or any other family member?  You have to take every patient’s situation differently.  If a patient smokes less than one pack of cigarettes a month, I would recommend waiting eight weeks for any surgery.  For a heavier smoker, I will not preform surgery for 4-6 months, and for very heavy smokers I will sometimes wait for one year.  In all of my patients with a history of smoking, I will get a lab test before the surgery to ensure that there is no nicotine in the blood system.

Marijuana can effect the outcome of a facial plastic surgery.  Marijuana can effect the healing and the effectiveness of the anesthesia during a facelift, rhinoplasty, or any other facial plastic procedure.  It is important to tell your physician if you do smoke marijuana or have taken any other substances, legal or illegal.  You cannot get in trouble or be arrested for telling your physician this information.  Please tell your doctors about these substances so they can keep you safe!

New Medication Combination Reduces Nausea After Cosmetic Procedures

Dr. Kulbersh is always reading current litearture to stay on the cutting edge and improve the outcomes and experiences for his patients.  The most common unpleasant issue after facial cosmetic surgery is nausea or even vomiting.
